List your participation in contests, fairs, shows and other events where your 4-H work is being evaluated and or recognized. If a column is not applicable just add N/A.
DATE - You may wish to use just the year, month and year, or the full date including month, day, and year. You determine which is the most appropriate for each individual experience.
LEVEL - indicate the level corresponding to your involvement.
· L = Local - activity sponsored by one or more 4-H clubs.
· C = County - activity sponsored by county.
· A = Area - activity sponsored to involve more than one county.
· S = State - activity sponsored at the state level.
· R = Regional - activity sponsored to involve more than one state.
· N = National - activity sponsored by National Council.
· I = International - activity including areas outside the U.S.A.
PROJECT CATEGORY CODES - You must identify the project category code. Indicate the code number for the category in which you are submitting this report.
